New Endodontics Patient Information

Hello and welcome to our practice! Please see the information below regarding your fees and new patient paperwork.

1. If you haven’t received a call already, please call us for your fee for your appointment. This is due day of service. We accept Credit/Debit Card, HSA, HRA, Flex, and Cashiers check (Credit cards only will have 3% surcharge). We are also pleased to offer simple financing options through Cherry – https://pay.withcherry.com/meridianendo
2. Fill out the Patient Health History by using this link (if you cannot complete this prior to coming in, please arrive 30 min early)

3. Send us a picture of your insurance card to office@meridianendo.com, ideally 24 hours prior to your appointment for an accurate estimate.
