New Periodontic Patient Information

Hello and welcome to our practice! Please see the information below regarding your fees and new patient paperwork.

1. Please fill out the Patient intake form by using this link (if you cannot complete this prior to coming in, please arrive 30 min early)

2. Normally our exams and images do not exceed $475.00 for In-Network Dental Insurance and $550 for Out of Network Dental Insurance and payment is due at the time of the exam. We will bill your insurance after your exam, and should they cover any of your visit you will be reimbursed.
